This 20 mg vial of retatrutide (LY3437943) was not assayed in the ordinary way. The sample went to the laboratory without a label, and the laboratory had to work out what it was before it could say how pure it was.
That distinction is the whole point of this page. In a standard third-party assay, the vendor tells the laboratory the target compound, and the laboratory reports purity against that target. The test answers one question — how much of the sample is the thing you said it was — and it takes the identity claim on trust. A blind test withholds the answer. Lot CS-re20-0710 was submitted on a common GLP-1 peptide panel spanning semaglutide, tirzepatide and retatrutide, with no indication of which it was supposed to be. Janoshik Analytical identified it as retatrutide and reported it at 99.643%, with 24.51 mg of net peptide against a 20 mg label, under report #207689 dated 30 July 2026.
Be clear about what that does and does not establish. It does not make the purity number larger or more precise. It makes the identity claim independent — the laboratory reached “retatrutide” from the chromatogram and the mass, not from a line on a submission form. Sequence-adjacent substitution is the failure mode this catches: the assay that most plausibly goes wrong in this category is not one that fails to detect impurity, it is one that confirms a compound because it was told which compound to look for. Our 30mg lot was tested the same way. Our 10mg lot was not — it holds a Testides certificate instead, and we do not describe it as blind tested.
Retatrutide itself is CAS 2381089-83-2, formula C221H342N46O68, average molecular weight 4731.33 Da: a single 39-residue lipidated peptide that agonises three receptors at once — GIP, GLP-1 and glucagon. At $140.00 for the vial it works out at $7.00 per labelled milligram, a 22% saving against the 10mg vial, and it is the middle option in a three-size range where the differences that actually matter are which laboratory tested which lot and what kind of test it was.
And the frame that no amount of analytical evidence changes: retatrutide is investigational and approved nowhere. Health Canada has authorised no retatrutide product, there is no Drug Identification Number, and Health Canada named retatrutide explicitly in its public advisory of 9 April 2026 on peptides bought online. A certificate tells you what is in the vial. It tells you nothing about whether the compound has been through a safety review, and this one has not.

Start with what a standard third-party assay actually is. A vendor sends a sample to a laboratory with a submission form that names the compound. The laboratory develops or selects a method for that compound, runs it, and reports the area-percent of the expected peak plus, if quantitative content was ordered, the milligrams present. The report is honest and the laboratory has done nothing wrong. But the identity claim in that report originated with the vendor, and the analysis was designed around it. A blind test removes the answer from the question. Lot CS-re20-0710 went to Janoshik Analytical on a common GLP-1 peptide panel — semaglutide, tirzepatide, retatrutide — without being told which of the three it was. The laboratory had to resolve identity first and assay second. It returned retatrutide, 99.643% pure, 24.51 mg net content, report #207689, 30 July 2026. The two questions, separated Standard assay: “This is retatrutide — how pure is it?” The purity figure is independent. The identity is asserted. Blind test: “What is this, and how pure is it?” Both answers are independent. The laboratory's conclusion is generated evidence rather than confirmed input. Why that gap is worth closing in this specific category. The realistic adulteration risk with GLP-1-family peptides is not an obviously wrong powder — it is a cheaper, closely related peptide from the same family sold as the expensive one. Semaglutide, tirzepatide and retatrutide differ enormously in price per milligram and are all long-chain acylated peptides that look broadly similar on a poorly chosen method. If the laboratory has been told what to expect, a method tuned to that expectation can return a high, clean, entirely truthful purity figure for the wrong molecule. Running the three as a blind panel is precisely the design that forecloses that. What a blind test is not. It is not an accreditation, it is not a compendial identity test against a pharmacopoeial reference standard, and it does not make 99.643% a better number than 99.643% obtained any other way. It also does not certify the next lot: blinding applies to the sample that was submitted, and every new lot has to earn its own report. And it is not a Testides certificate. Because vials are overfilled on purpose, and because content and purity are two unrelated measurements that vendors routinely blur together. Overfill is ordinary manufacturing practice. Lyophilisation and filling both lose material — to vial walls, to the stopper, to fill-volume tolerance, to residue that never leaves the container. A fill set to the exact label weight would put a real fraction of vials below label. A fill set above it does not. All three of our retatrutide lots measured above label: 11.88 mg against 10, 24.51 mg against 20, and 34.20 mg against 30. Net peptide content is not a purity figure. Content is a quantitative mass determination answering “how many milligrams of retatrutide are in this container”. Purity is an area-percent ratio from the chromatogram answering “of the peptide material present, what proportion is the target”. A vial can be 99.7% pure and short-filled; it can be generously filled and 90% pure. Presenting a favourable content figure as evidence of quality is a category error, and multiplying the two together produces a number that means nothing. We price by label weight. $140.00 for the vial, $7.00 per labelled milligram. The 24.51 mg is what this lot contained when it was assayed on 30 July 2026, not a specification we can promise for the next one. When the next lot is tested we will publish whatever it measures.
One molecule, three receptors, all of them class-B1 G-protein-coupled receptors that signal through G s and cAMP. Retatrutide is not a blend of three peptides and it is not a formulation — it is a single 39-residue chain, CAS 2381089-83-2, with a C20 fatty diacid attached to Lys17 through a γ-glutamate and two AEEA spacers. The lipid binds serum albumin reversibly, which is why the phase 1b terminal half-life was about six days rather than minutes (Urva et al., Lancet 2022;400(10366):1869-1881, PMID 36354040). GIPR — glucose-dependent insulinotropic polypeptide receptor Glucose-dependent potentiation of insulin secretion, effects on adipose nutrient handling, and central contributions to intake and to nausea tolerability. This is the arm at which retatrutide is most potent — roughly 8.9-fold above native GIP. GLP-1R — glucagon-like peptide-1 receptor Central suppression of food intake, slowed gastric emptying, glucose-dependent insulin secretion. Retatrutide is about 0.4-fold as potent here as native GLP-1 — weaker, not stronger. GCGR — glucagon receptor Hepatic lipolysis and fatty-acid oxidation, substrate mobilisation, raised resting energy expenditure — and, as a liability, raised hepatic glucose output. About 0.3-fold as potent as native glucagon. Those relative potencies come from cryo-EM structures of retatrutide bound to each of the three receptors (Li et al., Cell Discov 2024;10(1):77, PMID 39019866). The same work shows how one peptide manages three pockets: a single continuous helix with residues 1–13 in the transmembrane core, conserved contacts doing the shared work — an N-terminal tyrosine salt bridge, aromatic stacking, hydrophobic contacts at Tyr10 and αMeLeu13 — and receptor-specific accommodations, including a flexible proline-rich first extracellular loop in GIPR and an outward shift of about 4.6 Å in GCGR's seventh transmembrane helix. The practical consequence: retatrutide is a GIPR-leading agonist. It is not a uniformly stronger incretin. An assay normalised only to a GLP-1R readout will systematically understate its activity, and a design that assumes equipotency across the three arms will misattribute effects. The discovery and preclinical pharmacology are in Coskun et al. ( Cell Metab 2022;34(9):1234-1247.e9, PMID 35985340).
The usual answer is “one more receptor, so more effect”. That is not the mechanism and it misses what is genuinely novel here. Tirzepatide's two arms both act on the intake side. GIPR and GLP-1R agonism reduce food intake centrally, slow gastric emptying and potentiate insulin secretion. Add them together and you get more of the same kind of effect, which is why the dual agonist's efficacy and its adverse-event profile are both dominated by appetite and gastrointestinal phenomena. Retatrutide's third arm acts on the expenditure side. Glucagon receptor agonism works on the hepatocyte: it drives lipolysis and fatty-acid oxidation, mobilises stored substrate and raises resting energy expenditure. It is not additive in the same direction — it operates on the other half of the energy-balance equation. For most of the history of diabetes pharmacology glucagon was the hormone you tried to block, because its other effect is raising hepatic glucose output. The reappraisal that led to deliberately agonising it is reviewed in Elmendorf et al. ( Pharmacol Res 2026;223:108077, PMID 41478576). The design premise is that the incretin arms neutralise the glucagon arm's liability. GIPR and GLP-1R agonism lower glucose; GCGR agonism raises hepatic glucose output; the fixed stoichiometry of a single molecule is the bet that the balance lands favourably. That is a real bet with a real failure mode, and it is why the diabetes data function as a safety readout as much as an efficacy one. In the phase 2 diabetes trial the balance held: HbA1c was measured at 24 weeks in 281 adults, against dulaglutide as active comparator, with no severe hypoglycaemia reported (Rosenstock et al., Lancet 2023;402(10401):529-544, PMID 37385280). The readout where the third arm shows most clearly is liver fat. In a phase 2a trial of 98 participants with metabolic dysfunction-associated steatotic liver disease, relative liver fat was measured at 24 weeks across escalation arms against placebo (Sanyal et al., Nat Med 2024;30(7):2037-2048, PMID 38858523). The authors are explicit that the reductions tracked with weight and abdominal fat change, so this is not a clean glucagon-specific dissection — but hepatic steatosis is exactly what GCGR agonism predicts. Running this vial against tirzepatide and semaglutide is how the contribution of each added arm gets isolated.
Phase 2 in obesity — the trial most often cited without a checkable reference. Jastreboff AM, et al., N Engl J Med 2023; 389(6):514-526, PMID 37366315, NCT04881760. Randomised, double-blind, placebo-controlled, 338 adults, 48 weeks, escalating weekly doses against placebo, with least-squares mean body-weight change as the primary measure. Anyone citing “Jastreboff, NEJM, 2023” without volume, pages or PMID has given you a reference you cannot check. Phase 2 in type 2 diabetes — Rosenstock et al., Lancet 2023;402(10401):529-544, PMID 37385280. 281 adults, 42 US centres, placebo- and dulaglutide-controlled, measuring HbA1c and body weight at 24 weeks. A body-composition substudy followed (Coskun et al., Lancet Diabetes Endocrinol 2025;13(8):674-684, PMID 40609566). Phase 2a in steatotic liver disease — Sanyal et al., Nat Med 2024;30(7):2037-2048, PMID 38858523, n=98, figures above. Phase 3. The obesity programme is TRIUMPH and the diabetes programme is TRANSCEND. TRIUMPH-1 (NCT05929066, 2,339 participants, 80 weeks) was reported by the sponsor on 21 May 2026, with a pre-specified extension to 104 weeks in 532 participants. TRIUMPH-2 (NCT05929079, n=1,152, type 2 diabetes) and TRIUMPH-3 (NCT05882045, n=1,949, severe obesity with established cardiovascular disease) were reported on 23 July 2026. TRIUMPH-4 (NCT05931367, n=445, obesity with knee osteoarthritis, 68 weeks) reported on 11 December 2025. The first phase 3 result to reach peer review is TRANSCEND-T2D-1 (Bajaj et al., Lancet 2026;407(10546):2402-2413, PMID 42250575, n=537, 40 weeks), which measured HbA1c and body weight against placebo. A chronic kidney disease trial is running, with its design and baseline paper published (Heerspink et al., Nephrol Dial Transplant 2026;41(6):1058-1068, PMID 41160422). Read the sourcing. Everything above tagged with a PMID is peer-reviewed. Everything tagged with a date and an NCT number but no PMID is a sponsor press release — no peer review, no supplementary appendix, no independent scrutiny of the analysis. We label the difference because most pages in this category do not.
Every published endpoint is a surrogate. Body weight, HbA1c, liver fat fraction, blood pressure, lipids. No completed cardiovascular outcome trial, no renal outcome trial, no mortality data. TRIUMPH-3 enrolled people with established cardiovascular disease and reported weight and safety, not events. Longest reported exposure is 80 weeks, with an extension to 104 weeks in a subset. No multi-year safety dataset, no pregnancy data, no paediatric data. There is a heart-rate signal, and it is quantified. In the phase 2 obesity trial heart rate rose dose-dependently, peaked at 24 weeks and then declined. A 2026 network meta-analysis in non-diabetic people with overweight or obesity puts retatrutide at +3.46 bpm (95% CI 1.74 to 5.18), against semaglutide +3.35, tirzepatide +2.05 and orforglipron +7.30 (Zhang et al., Eur J Med Res 2026;31(1):318). Blood pressure and lipid fractions were also pooled in that literature, with no significant HDL-C effect reported (Simental-Mendía et al., High Blood Press Cardiovasc Prev 2026;33(5):503-516, PMID 42371360). What a sustained chronotropic effect means over years is unresolved. Tolerability is dose-limiting. In TRIUMPH-1 the sponsor reported nausea in 28.6–42.4% of treated groups against 14.8% on placebo, diarrhoea 25.2–34.1% against 13.5%, vomiting 10.6–25.3% against 4.8%, and discontinuation for adverse events at 4.1%, 6.9% and 11.3% against 4.9%. In TRIUMPH-4 discontinuation reached 18.2% on the top dose. Two unexplained signals are specific enough to name: dysesthesia (5.1–12.5% in TRIUMPH-1, up to 20.9% in TRIUMPH-4, against under 1% on placebo) and a urinary tract infection imbalance (7.5–8.8% against 5.3%), the latter now the subject of published commentary in the European literature. Discontinuation has not been studied for retatrutide. We could find no published randomised-withdrawal or off-treatment follow-up for this compound and will not manufacture one by analogy. What the class shows: the STEP 1 extension followed participants to week 120 after semaglutide withdrawal and recorded regain of weight lost, against placebo (Wilding et al., Diabetes Obes Metab 2022;24(8):1553-1564, PMID 35441470); SURMOUNT-4 randomised participants off or on tirzepatide at week 36 and followed both arms to week 88 (Aronne et al., JAMA 2024;331(1):38-48, PMID 38078870). And the mechanism of the third arm has not been isolated in humans. Whether the liver-fat effect is glucagon-receptor-driven or a consequence of weight change is not settled by the published trials, because the trial designs cannot separate them. That is an open question, not a solved one, and it is one of the more interesting things this reagent is for.
No DIN, no authorisation, and named by Health Canada. No retatrutide product has ever been authorised for sale in Canada, so no Drug Identification Number has ever been issued and there is no DIN history to report. As of September 2026 retatrutide is not approved by any regulator anywhere in the world — not Health Canada, not the FDA, not the EMA. The developer describes it as investigational. The advisory names it. Health Canada's public advisory of 9 April 2026 — “Think twice before injecting peptides bought online: unauthorized products can seriously harm you”, identification number RA-81874 — lists retatrutide alongside BPC-157, CJC-1295, DSIP, Epitalon, GHK-Cu, hCG, ipamorelin, KPV, melanotan I and II, MOTS-c, NAD+, SS-31 and TB-500. Health Canada states these products have not been assessed for safety, efficacy or quality and that they are illegal in Canada, names hormonal imbalance, blood-sugar disturbance, liver or kidney damage, blood clots and tumour growth among the risks, and states directly that labelling a product “For Research Use Only” does not make it legal. We reproduce that rather than bury it. This vial is sold as a laboratory reagent and not for injection into anyone for any purpose. The blind test addresses exactly one of Health Canada's concerns — whether the contents are what the label says — and none of the others. A correctly identified, 99.643%-pure compound that has never been through a regulatory safety review is a correctly identified, pure, unapproved compound. What is coming, and what that does not mean. On 23 July 2026 the sponsor stated it plans to submit retatrutide to the FDA in the first quarter of 2027. A filing is not an approval, review timelines move, and a United States submission is not a Canadian one. There is no announced Health Canada submission date that we could verify, and we will not invent a timeline for one. WADA. Retatrutide is not named on the WADA Prohibited List, and GLP-1 and glucagon receptor agonists do not appear as a prohibited class. WADA placed this drug class on its Monitoring Program instead — semaglutide from 2024, tirzepatide from 2025 — and confirmed in September 2026 that they stay off the Prohibited List for 2027 while performance studies continue. We could not verify that retatrutide is named individually on the Monitoring Program and do not claim it is. Monitoring is not permission and the list is revised annually.

The argument against always buying larger: a bigger vial concentrates the project into one reconstitution event and one set of aliquots, and every freeze-thaw cycle after that costs material. Lyophilised powder in a sealed vial is the stable form. Work spread thinly over many months is often better served by several small vials than by one large one, whatever the per-milligram arithmetic says.
Sealed, desiccated and lyophilised: −20 °C for long-term holding, 2–8 °C for short-term, protected from light. A sealed vial tolerates ambient shipping temperatures for the transit period without measurable loss; the cold requirement is about holding, not about the journey. Why the convention is conservative. A licensed peptide drug product is a formulation with buffer, bulking agent and sometimes a surfactant, backed by a stability programme and a shelf life. This vial is neat peptide with no excipient and no shelf-life study behind it. Nothing in the container is protecting the molecule from moisture uptake, oxidation or interfacial stress, so the sensible default is the cold, dry, dark one. Structural liabilities worth knowing. Retatrutide carries no methionine and no cysteine, so methionine oxidation and disulfide scrambling — the two classic routes for many peptides — do not apply. It does carry aspartate and asparagine-adjacent positions where deamidation and isomerisation are the usual degradation chemistry for this family, and a long C20 fatty diacid that makes the molecule surface-active, so adsorption to glass and plastic and aggregation at air-liquid interfaces are realistic losses. Avoid vigorous agitation and avoid leaving dilute solutions standing in untreated containers. These are inferences from the sequence, not conclusions from a published stability study on this compound, and we present them as reasoning rather than as cited data. A 20 mg vial makes aliquoting more important, not less. More material in one container means more of the project riding on a single reconstitution. Divide into single-use aliquots at first preparation so no working volume is thawed twice, and do not return thawed material to the freezer. We stock bacteriostatic water 10ml and 3ml for laboratory reconstitution. We publish no figure for how long reconstituted retatrutide stays intact, because we could not find a verifiable published stability study for this peptide in solution at any concentration, buffer or temperature. A supplier quoting a specific number of days is quoting something nobody has measured.
